MINISTRY OUTREACH REQUEST FORM

Greetings UW Partner in Ministry,

Outreach is important to the Lord Jesus Christ. He said in Matthew 25:37-40 “…when did we see you hungry and feed you, or thirsty and give you something to drink? When did we see you a stranger and invite you in, or needing clothes and clothe you? When did we see you sick or in prison and go to visit you?’ The King will reply, ‘Truly I tell you, whatever you did for one of the least of these brothers and sisters of mine, you did for me.”  It is for this reason that the Lord expects us to do our best to meet the basic needs of others. 

The Union Wesley Outreach Ministry (UWOM) invites you to submit your outreach request on this Outreach Request Form.  The form is designed to capture your outreach request and assist in partnering, planning, and addressing needs of Health, Homelessness, Food Insecurity and Human Development in the Ward 5 community and the needs of neighbors that expand beyond our borders.
